Name and address of person making the request.

Date of the request.

The nine digit CASE NUMBER found in the upper left corner of the Accident Information Summary.

Note: YOU MUST INCLUDE THIS NUMBER!

City or town where accident occurred.

First and last names of operator(s) of motor vehicles involved in accident.

A check or money order in the amount of $8.00, payable to the Commissioner of Public Safety.
